Health & Welfare
From date of hire you serve a 30 calendar day probationary period (or in accordance with your contract). On the 31 st day, your employer begins to submit weekly Health and Welfare contributions to the Central States Fund. Once your employer has paid eight consecutive weeks of contributions on your behalf (on the ninth Sunday following your probation) you and your family have coverage and your cards and books will be mailed to you.
Note: If you do not work at least 1 day or tour each week, Sunday through Saturday, your employer is not required to pay, and you should contact Local 24 to self-contribute in order to establish eight consecutive weeks. You must go to a TeamCare provider for full coverage, this means physicians and hospitals in the TeamCare network (PHCS). You pay a $20.00 co-payment towards the office visit and the remaining office visit charge is covered by your TeamCare benefits. There is a $200 per person, $400 per family plan deductible. Major medical charges are covered at 80% after deductible. You can view your plan information at www.centralstates.org . National Master Freight Companies are under Plan C6, all others covered by Central States are on Plan C4.
You must complete the enrollment form mailed to you by Central States Health and Welfare Fund as soon as possible after receiving it in order to add your spouse and dependents to your coverage. Family Protection Plan: In the case of a member's death, TeamCare provides coverage up to 5 years to the surviving spouse and dependent children under 19 years of age provided you stay within the network. Dentists, Optometrists, Chiropractors, Audiologists, Psychologists and Psychiatrists and Outpatient Rehabilitation for Drug and Alcohol do not come under the TeamCare benefit but are covered by your Central States C-6 Benefits. You may go to the provider of your choice in these areas. The OCT insurance becomes active immediately following your probationary period, usually 60 or 90 days. Once the OCT receives contributions on your behalf they will send out cards and books, along with an enrollment form.
This form must be filled out and returned immediately in order to cover your dependents. The deductible for this plan is $200 per person, $400 per family. There is no website for Ohio Conference of Teamsters Insurance, so any questions can be directed to Chris at the Union Hall or to the number on your card.. All other members of Local 24 will have insurance coverage through their employer.
