NEW MEMBERS HEALTH/WELFARE COVERAGE ACCIDENTAL DEATH AND DISMEMBERMENT

Payment of dues provides you with the following Accidental Insurance Benefit, in addition to any other coverage you have under contract. This is provided at no cost to you. This benefit is provided to you through “American Income Life Insurance Company” – a union company. The principal sum of $2000.00 is payable upon the following:

• Loss of life, loss of one hand and one foot, loss of both hands, loss of one hand and sight of one eye, loss of both feet, loss of one foot and sight of one eye. One-half the principal sum is payable upon loss of one hand, loss of one foot, loss of sight of one eye. In addition, a $500.00 accidental benefit is provided for your spouse and $250.00 for each of your children under the age of 19.
• This is an accident only policy and does not pay benefits for loss due to sickness.
