NEW MEMBERS HEALTH/WELFARE COVERAGE VISION BENEFIT

YOU MUST CALL FOR AN APPOINTMENT
This benefit is provided to those members covered by Plan C-6 of the Central States Health and Welfare Plan, and by Plan 6 or the 80/20 Comprehensive Plan of the Ohio Conference of Teamsters & Industry Health and Welfare Plan. The Vision Benefit covers eye examinations, lenses, frames, contact lenses and service for lenses and frames for you and your dependents as follows:

One eyeglass examination per person is covered every 12 months with a $10.00 co-pay. One pair of lenses per person is covered every 12 months. One set of frames or one repair to frames per person is covered every 12 months.
Only one pair of glasses (lenses/frames) or one set of contact lenses (basic single vision soft lenses) per person is covered every 12 months.

(Full-time students 19 or over are not covered by this benefit.)

Note: All exclusions are the same as Central States C-6 coverage, and if C-6 Vision Benefits change in the future, this plan will be adjusted. All other members of Local 24 not covered by Central States Plan C-6, Members of the Retired Teamsters Fellowship Club and full-time students 19 or over, the following fee schedule will apply:

Exam $35.00 for glasses Single Vision RX start at $45.00 (clear plastic) Bifocal RX start at $55.00 (with line, clear plastic) Progressive (no-line) start at $140.00
Replacement frames $40.00

Contact Lenses Exam starting at $45.00, fitting fee starting at $25.00, lenses – prices vary per type of lens, maintenance kit and follow-up visits) Photogrey and tints are an additional charge.

YOU WILL NEED TO SHOW YOUR LOCAL 24 MEMBERSHIP CARD OR RETIREES FELLOWSHIP CLUB CARD.
